NURS 414 Patient Case Mapping ® Assignment
This is a part of a two-part assignment involving the assigned case study describing the care of Ms. Diaz.
Case Map ® (15% weighted grade) . *Articles to be used and cited should be from 2016 -2021*
1. The case map® portion of the project includes the development of a case map to include the patient’s diseases identified in the case study. These case maps® are due by 1600 ON October 20, 2021
a) The diseases mapped include:
1) Bronchitis
2) Recurrent DVT
3) Diabetes Mellitus Type 2
4) Hyperlipidemia and Atherosclerosis
5) Hypertension
6) Coronary artery disease and MI
7) Acute Heart Failure and pulmonary edema with a new occurrence of atrial fibrillation with rapid ventricular response.
b) The pathophysiology of each condition,
c) The physical assessment related to that problem and seen prior to discharge,
d) Diagnostic tests reported during the hospitalization and related to that problem,
e) Medications and other therapies used during the hospitalization,
f) Nursing Care issues related to that problem at the point of discharge
2. The linkages between the pathophysiology of each disorder and the specific link to the diagnostics, assessment findings, nursing care and medications should be evident in the case map.

3. These are to be submitted to the blackboard assignment tab in one of two formats:
a) As an electronic flowchart product saved and submitted as a JPEG or with an accessible link provided to the faculty. The student (s) name(s) must be readily visible on the submission
b) An APA style reference page is also submitted as a separate document and includes an APA style (6th ed) cover page. There is no need for “in text parenthetical references” within the case map ® (only the APA-styled reference page).
