Sport Psychology

Requirements:
1. Patient/client summary: 3-5 pages
a. What brings the patient/client in for consultation?
b. Who is the patient/client? (i.e. personal background, age considerations,
home life, sport history, academic involvement)
i. How might the patient/client’s “story” impact the ways in which
they interact with you [the clinician]? (i.e. lack of support from
family in regards to return-to-sport following injury)
ii. What’s unique about the patient/client’s sport or injury history and
how do these things influence your work? (i.e. individual v. team
sport, acute v. chronic injury, special demands of the sport)
c. What are the patient/client’s goals in working with you?
2. Ethics: 1 page
CITATION REQUIREMENT: must include at least one scholarly resource
a. What unique ethical considerations are there given the setting in which
you are seeing the patient/client? (i.e. in the Athletic Training Room with
others present, one-on-one sessions on campus, etc.)
3. One weekly process note — approx. 1 page per note
Below are broad guidelines for SOAP note-taking. You may use this
format, or whichever format you see fit. Please be sure to include the
following in some capacity.
a. Subjective — what does the patient/client express?
b. Objective — what do you, as the clinician, objectively observe during your
session (i.e. current status, interventions used)?
c. Assessment — what is your professional take on the session? …what
informed your decision-making process in regards to things like
interventions used, etc.?
d. Plan — what’s your “game plan” moving forward?
4. Sport psychology inventory (psychometric / outcome measure) and explanation:
1 page
CITATION REQUIREMENT: must include at least one scholarly resource (citing
the outcome measure used)
Based on the patient/client’s “story,” what outcome measure (THINK:
survey or scale) would you use throughout your time together and why?
(i.e. client is being seen for general mental toughness training — Mental
Toughness Inventory, Coping Skills Inventory v. client is being seen for
readiness to return following ACL reconstruction — ACL-RSI) How often
would you assess your client’s progress and why?
5. Mental toughness tools and explanation: ½ page per explanation
CITATION REQUIREMENT: must include at least one scholarly resource per
tool (= total of 4)
Based on the patient/client’s “story,” you must design a minimum of 4
mental skills tools to use in session with the client. You must then provide
a ½ page explanation as to what concept within sport psychology the tool
addresses (i.e. affirmation development worksheet = self-talk and
confidence; goal setting game = self-efficacy and achievement)
6. Termination summary: 2-3 pages
At the conclusion of the semester, you will “end” your patient/client
sessions.
a. What are your observations in terms of client progress over the
semester?
b. What do you feel still needs addressing?
c. What are your recommendations for the client moving forward?
d. What have you learned about yourself as a clinician over the course of
the semester? …and how will this impact your consultations down the
road?
