Project 3.1 on page 111 asks you to compose a referral letter using the information given in the project, starting on paragraph two. You are creating the
referral letter for Dr. Larsen to sign, so remember to put the reference initials at the end of the letter. Working Paper (WP) 5 shows the ophthalmologist’s
complete address. Use the Chart Note in WP 6 on Florence Sherman to describe the situation to Dr. Diangelis.
Use the information on pages 97-98 and the style of the letter on page 99 to format the letter correctly. Create a Word document using the file name Florence
Referral-xx where the xx is your initials. When you have completed the letter, click on the Submit Assignment button on this page and attach the file to this
assignment. You do not need to submit the envelope or chart note. If you need more information on uploading files to Canvas, the instructions are in the
“Finding and Submitting Assignments” section in Module 0.
For this assignment, disregard the instructions in the first and last paragraphs to create an Outside Services file. Quite honestly, if you can’t find a file folder
and put a piece of paper in it, you might not belong in a medical office (LOL).
Section 2
In this project, you will be editing and proofreading two reports. I have attached the two report files below.
In the Working Papers at the back of the book, WP 7 and WP 8 show the changes the physician wants made, including line spacing. Study the proofreading
marks on page 108 in the textbook. Some are obvious, but others, not so much. You must make the changes the physician wants. In addition, there are other
errors that need to be corrected. DO NOT RELY on Word’s Spell Check program to catch everything. Read carefully and make ALL changes that are necessary:
spelling, grammar, and punctuation.

To download these files, right-click on the file, then select Save Link As or Save Target to (depending on your browser). Put the files into a folder named CIS
79 Student Files. To edit each file, open it in Word, then use the Save As command to save it with xx at the end (your initials) so that you know this is your
working file, and not the original file. When you finish correcting all of the errors, save the file and submit it using the Submit Assignment button. Make sure
you attach the file you have changed, not the original data file.
(And yes, this is hard; and no, I don’t expect you to be perfect. Just do your best so that you can get an assessment of your current skills, and some ideas for
where you could use improvement.)
RUBELLA (GERMAN MEASLES)

Rubella (german measles) is a communicable viral disease characterized by diffuse punctate, macular rash. Rubella is a relatively benign viral illness un less
there is transplacental transmision. (Define the following terms: communicable, diffuse, punctate, transplacental, and macular.)
Etiology
Rubella is caused by rubella virus (Rubivirus) that is spread by air borne direct contact with nasopharyngeal secretion. This disease is communicable from
one week before rash appears to five days after the rash disappears. Rubella is most common in children but may also affect adults who were not infected
during childhood. (Define the following terms: airborne, direct contact, and nasopharyngeal.)
Incidence
Rubella occurs most often in the spring, but there are major epidemics occurring in 6- to 9 year cycles. (Investigate recent epidemics vs. the use of the
vaccine.)
Pathophysiology
The virus invades the nasopharynx and travels to the lymphglands, causing lymphadenopathy. Then in 5 to seven days it enters the blood stream stimulating
an immune response causing the rash. This rash lasts about three days. (Define lymphadenopathy.)
Clinical Symptoms
Clinical symptoms of rubella include swollen gands, fever, sorethroat, cough, and fatigue. The pruritic rash generally starts in 1 to 5 days after the prodrome.
The rash begins on the face and trunk and spreads to the upper and lower extremities. Symptom of headache and conjunctivitis may occur after the rash.
