Your plan should include your education and resource materials as appropriate. Your plan should be culturally and age appropriate for your target population. Include how your key stakeholders will be involved. Be sure to include the health behavior model that is the basis for your health education plan and why you chose that model. How do you plan to reach your target audience? What is your message to get your target audience to participate in your health education plan? Is there any incentive? Include marketing materials as appropriate, ensuring they are culturally and age appropriate for your population.
DESCRIBE EXPECTED OUTCOMES – What do you anticipate to happen as a result of your target population engaging in your program? What does successful program look like? How will you measure the outcomes? Include any pre and post assessments you plan to include as part of your health education plan.
GRAMMAR, SPELLING, AND APA FORMAT – You should utilize 12 point font, double spacing, and APA format for the written project. Be sure to include citations. The paper should be 3-5 pages, not including any reference materials that will be used as part of your health education plan.
