A central purpose of the project is to provide the opportunity for you to apply the nursing process to a
community. You will be using a systematic and comprehensive approach in collecting, analyzing, and
summarizing primary and secondary data, which will lead to the identification of a community diagnosis (a
target population and related public health issue of the City of Quincy, Massachusetts) and of public health
nursing interventions at the individual/family, community and systems levels.
Please note that required data points have an asterisk beside them. For key assessment data points, you
will provide comparison data from the state level, from Quincy (Massachusetts) if you are assessing a
Quincy Neighborhood or from another city. You will identify 2 vulnerable populations and 1 public health
problem for each.
In addition, you will include the comparison data and a draft of the comparison charts/tables and
identification of two vulnerable populations and a related public health problem for each.
Remember that you are not just listing data but should be presenting some analysis of the data (what does
it mean).

Comparison data is extremely important in understanding the context of a specific community. For at least
3 Key demographic data points related to the social determinants of health(racial/ethnic breakdown,
income levels, including those living at the poverty level, educational attainment, unemployment rate )
identify comparison data at the state level and with Boston or another city (using percentages not raw
numbers) and present these comparisons in data charts/tables (as appendices) with some analysis
regarding the data the body of the paper. Three charts/tables are required, but it is useful to include
throughout the body of the paper any comparison data that you find.
Identify 2 vulnerable populations in the community. Vulnerability can be based on key attributes such as
age, race/ethnicity, SES, ableness, gender or sexual orientation, or membership in a group/population such
as homeless or veteran. Explain why are they vulnerable? Why are they at risk?
Identify and discuss one public health problem/issue for each vulnerable population
Based on the data, what population and public health issue will you focus on, for example, high school
students and vaping; older adults and depression; school-age children and vaccination rates; homeless
population and substance use disorder
