Answer the following short answer questions. You may use any health behavior/problem of your choice – But you may not use examples used in class and in
readings. You can use different health problems/behaviors for different questions, or the same one.
This is an independent assessment. You are to work on this alone. You cannot work in groups, receive help from classmates, or utilize online materials that
were not assigned by the instructor (e.g., Google, Wikipedia, online test banks, etc.). You are allowed to use course materials (lecture slides, readings, etc.).
As a reminder, plagiarism, cheating, or other forms of academic dishonesty will not be tolerated and can lead to sanctions up to and including separation
