Along with the above questionnaire, please attach and turn in the following:

1. Handout of Step-by-Step Protocol for Caregivers/Staff
a. Write this in a directive tone as a protocol you would give to the caregiver that tells them what to do. This may be in bullet format, or as a flow chart of what to do when, or as a step-by-step instruction guide. This should include, but is not limited to: materials needed, antecedent conditions (e.g., SDs, preference assessments, etc.), what to do for correct responding (i.e., praise and reinforcement), what to do for incorrect responding (i.e., error correction procedures), data collection (procedures for data collection should be described in the data sheet component). This should be written in language that the caregiver will understand, free from ABA jargon.
2. Targe Behavior Data sheets:
a. Ceate a data sheet for the caregiver /staff to use for collecting data on the client’s progress and instructions (written to the caregiver/staff) for how to fill out the data sheet.
b. Create a data sheet to collect procedural integrity to measure implementation of the protocol and instructions for how to score the procedural integrity data sheet, with criteria for additional training.
3. Social validity questionnaire
a. Create a social validity survey. Questions should evaluate the acceptability of: (1) goals, (2) procedures, and (3) outcomes.
