Interprofessional Collaborative Practice
Purpose of Activity: The purpose of this activity is to develop a real-world plan of care as a member of the interprofessional team. During clinical experiences,
you should have had the
opportunity to engage in interprofessional collaborative practice.
Directions:
Using your selected documentation reflect on the patient’s care and interprofessional team.
– Select one sub-competency from one of the core competencies in which you felt was lacking from your patient’s plan of care but would have been beneficial
in their management.
– Develop a plan of care for how you would have addressed this sub-competency and the team members that would be needed.
Grading Criteria
– Provide a brief synopsis of the patient. No longer than one paragraph.
– Identify the sub-competency.
– Discuss three to four points in the plan of care to address the sub-competency. At
least two of the points should derive from the literature/evidence.
– Discuss the team members needed and their role.
