Ms. PM is a 31-year-old G3P2 African-American female who presents to your clinic today complaining of unusual fatigue, nausea, and vomiting for the last five days. She has a medical history of chronic hypertension (HTN) that was diagnosed shortly after her first pregnancy two years ago and GERD. MS. BD’s blood pressure is controlled on Hydrochlorothiazide 12.5mg by mouth daily. During the interview, you learn that she is single, sexually active, has one partner and that her menses is ten days late. She performed a home pregnancy the three days after missing her menstrual cycle, and the results were inconclusive. She states she feels terrible and needs relief. She has no other medical problems, symptoms, or concerns.

Medication: HCTZ 12.5 mg daily

Current Vitals: 130/70, HR 68, Ht: 65 Wt: 200

HEENT: Denies any headache, nasal congestion, ear pain. Reports fatigue
Cardiovascular: Denies chest pain, swelling
Respiratory: Denies any SOB or DOE
GI: Reports nausea, vomiting, worseing reflux
GU: Denies any painful urination, urgency, hesitancy
Musculoskeletal: Denies
Neurological: Denies
Psychiatric: Denies suicidal ideations, depression. Does report difficulty falling asleep
Endocrine: Denies
Hematologic/ Lymphatic: Denies any bruising

OBJECTIVE:
Vital signs: Ht: 65, Wt: 200, BP 130/70, HR 68, Resp: 16

Constitutional: Well developed, overweight female

Cardiovascular: RRR, No murmurs or rubs noted. No peripheral edema noted
Respiratory: Clear bilaterally all fields.
GI: Abdomen round, soft, with bowel sounds noted in all four quadrants.
GU: Deferred
Integrementary: Skin warm and dry, No rashes noted
