For your assignment you will be completing nutrition assessment notes for 2 patients in EHR Go!. You will use the attached nutrition assessment template
and fill out as much information as possible based on what you find in the patient’s chart. You may be able to fill out a lot and you may not, that depends on
what you find in their chart. Keep in mind you will only be completing the top section for data within ‘A’, ‘B’, ‘C’, ‘D’, & ‘E’ only.
Hint! For both clients you WILL be able to find (at least the following):
A =height (ht), weight (wt), can calculate BMI and IBW (including range)
B = both have nutrition-related labs available
C = Admission diagnosis (Admit Dx), age & DOB, gender, ethnicity, patient history (Pt Hx)
D = Diet order (either at admit or once released), meal/snack pattern, food allergies, supplements
E = at least one piece of data for each patient in this section
